Step 2: Using the usage history block on a past Eversource bill, or the “green button” on the Eversource website, determine the plan level most appropriate.
Step 3: Complete the enrollment information page.
Step 4: Read and sign the purchase/sales agreement.
Step 5: Return completed enrollment form, the purchase/sales agreement, credit card authorization and a copy of your Eversource bill to Self-Reliance, PO Box 396, North Falmouth, MA 02556-0396 or fax 508-563-1123. As this contains credit card information we ask that you do not email the forms.
